Revoke Connection

Revokes every access/refresh token the caller holds for this client. Scoped to user.id, not anything the client supplies, so this can never touch another user's grant -- if client_id doesn't belong to the caller, revoke_grants_for_client's filter matches zero rows and this is a silent no-op, same 204 either way (no oracle for client existence).
