Get Portal Claim

Serve the full claim-detail report to an assigned guest (Portable Citations).

Reuses the exact same builder as the authenticated GET /claims/{id}, so the invited attorney sees a byte-identical report (merged extraction, inference steps, documents, citations) — just scoped by ClaimAssignment instead of org membership. Every hit is PHI-audit-logged with IP by the middleware.
