Submit Secure Info

Receive PHI fields (SSN, MBI, DOB, address) on a self-service MSA claim.

Auth is by HMAC token bound to the claim_id, sent in the post-payment email. The submitter never authenticates with a Zicron account — this is the only path for them to deliver the high-PHI fields without us putting them on the public order form.
