Respond To Adr

Generate an ADR response for a hospice claim and persist it.

Loads merged_extraction from the DB (if configured) or the in-memory store, re-scores hospice inference deterministically, runs the generator, and appends the response to workflow_state["hospice_adr"]["responses"] so the artifact survives server restarts and is retrievable for audit.
