Add an LLM API key.

Stores a provider API key for the organization (orgId). The plaintext key is write-only: it is encrypted at rest and can never be read back through the API — subsequent responses expose only metadata.

Returns 201 Created with the new key's metadata. Restricted to organization owners and admins (403 otherwise).
