Start3 Generate

Start3 essay generation via opus 4.8 agent.

Generates a title (opus 4.8), writes a title-only document to DB, then kicks off the start3_full_generation_task celery task which runs the opus essay agent with the wired length/depth/citation/library params. No fallback.
