Removed: raw order creation (answers 410)

Orders are created from a quote only: POST /agent/swap/quote, then POST /agent/swap/create with { quoteId } and an idempotency-key header. This older route made orders without a quote (origin other: never payable, payableReason unverified_origin) and now answers 410 raw_create_disabled. Reading orders is unchanged (GET /agent/swap/status/{orderId}, GET /agent/swap/orders).
