Generate a fresh JotForm reauth link

Return a pre-filled JotForm URL the partner can hand back to the patient to re-authorize their prescription. Use this when refills are exhausted or the prescription is expiring. The same URL is delivered automatically in the rx_renewal_needed webhook; this endpoint lets partners pull it on demand.
