Schedule a group coaching check-in

Creates a new check-in session record and books it via Calendly. Unlike 1:1 scheduling, the session is created at booking time. Only one active (scheduled) check-in per participant per cohort is allowed.

MED-699: Check-in type. Defaults to "general" (legacy behavior — creates a new session record). "intro" and "midpoint_checkin" update the pre-existing placeholder created by MED-697; the placeholder must be in status=unscheduled.
