Credit balance and storage usage

Everything needed to predict a 402 before you spend a request on one: the prepaid balance separation is charged against, how much storage is used against the limit, and the live per-minute rate. Separation costs 4 credits (cents) per minute of audio, or a flat 20 when you start a job without sending duration_seconds.
