Reextract Document

Re-run extraction on a stored document (restart recovery / a retry after a failed run, or a deliberate re-read under a new extraction_run_id — terms are append-only, supersede never erase). 409 {"error":"extracting"} while a run is already in flight. Spends the key, so it shares the docs spend bucket.
