Report an entry

Reports a problem with an entry to the site’s reviewers. It is the report page’s own form, so it answers as the form does: a 303 back to the page, ?sent=1 when the report was taken, ?error=<what> when it was not (reason: no such reason, or an image report on an entry with no image; note: an image report needs a note; blocked: the note has a word the site does not take). An image report names the image (media), or the lead image is meant.
