Store one in-app suggestion; 201 {"id": "fbs_..."}.

422 suggestion_empty / suggestion_too_long / suggestion_invalid for a refused message (a blank message passes pydantic's length check and is refused here), 429 suggestion_rate_limited with Retry-After when the principal already sent the window's maximum.
