Disconnect Slack Install

Disconnect the workspace: revoke the token with Slack (best-effort — the row is disarmed regardless), then clear the bot lane if it points at the installed channel, so the mirror stops trying a channel we can no longer reach. Webhook-lane fields survive (apply_bot_lane_cleared).
