Clear Scheduled Change

Cancel a scheduled plan change (e.g. an already-requested downgrade) before it applies at period end - pending_tier/pending_change_at go back to null and the caller keeps their current tier indefinitely (until they request another change). Does not touch cancel_at_period_end.
