Set Default Model

Set the organization's default AI model by catalog id.

Names aren't unique once an org enables the same model_name under multiple providers (e.g. openai/gpt-4o + azure_openai/gpt-4o), so the selection is keyed by catalog row. Rejects catalog ids that aren't enabled for this org with a 404.
