Submit a Termly Health Form

Requires permissions: Guardian or Parent

Submits a health form for one of the parent's children. The form is open all year: parents may submit whenever something changes, not only in a termly window. A submission that reports an actual change emails ALL of the pupil's house staff — no role filter, so deputies and tutors receive it alongside the housemaster and matrons — plus the Medical Centre.
