Confirm 2Fa

Activate TOTP 2FA. Supply the secret + ticket from /enroll and a code generated from that secret. On success 2FA turns on and the recovery codes are returned ONCE — store them (they're the only self-service way back in if the authenticator is lost; key recovery does NOT clear 2FA).

409 AUTH_2FA_ALREADY_ENABLED; 400 AUTH_2FA_INVALID (bad/expired ticket or wrong code).
