Query parameters

TBP-594 — optional. The controller runs every request through boundAppId(req, query.appId), which takes the app from the authenticated token and rejects a supplied value that disagrees with it. Requiring it here meant a caller holding a perfectly good app token still had to know and repeat its own app id, and the management SDK does not carry one — it has the key, not the claims. Omitted, the token's app is used; supplied and mismatched, it still 403s.
