Server-Sent Events stream of repo index-status changes (org-scoped)

Long-lived text/event-stream. Emits event: index-status frames whose data is {repoId,status,error,lastIndexedSha} for the caller’s org, plus a heartbeat comment every ~25s. Consume with a credentialed EventSource.
