为指定 MCP 服务下的某个工具更新启用状态、人工审批要求和/或风险等级。 至少提供 require_approval、enabled、risk 之一；省略的字段保持原值。 risk 为 read/probe/write/destructive，空字符串表示改回按服务注解推断。
