Permanently delete a document (admin)

Irreversibly hard-delete any manual and all associated pages, embeddings, and ManualModel rows, regardless of ingestion or visibility status. Intended for test fixture cleanup, failed-ingest removal, and administrative housekeeping. Requires the admin key.
