List Phone Console Calls

Every phone call this backend is holding, newest and live ones first.

The ring first (it has the frames), then every live row the record holds that the ring does not — which in production is all of them, since no instance's ring is armed there. A ring call is named by the carrier's id and a record call by its row id, so a row is dropped when either of its names is already in the ring's list.
