Snapshot of every registered circuit breaker.

Intended for ops dashboards + manual oncall checks. Returns the state (closed / open / half_open), failure counters, and last failure reason so we can tell at a glance whether any upstream dependency is currently degraded on this process.

Note: state is per-process. With multiple uvicorn workers you'll see one entry per worker; that's intentional (see breaker.py).
