Billing Dispatch Reconciliation

Reconcile billed Media Worker requests whose dispatch result was lost.

Schedule this endpoint every minute. Individual operations carry their own next_reconcile_at timestamp; stale reserved/dispatching operations are also recovered, so frequent invocations are safe and do not hammer the worker.
