Approve a skill approval request

Approves a pending request: a creation publishes the skill within its org/department/team scope, while a modification promotes the held version to latest. The reviewer must be an org ADMIN, the skill's department admin, or (when member approvals are enabled) any member, and cannot review their own request. Records an audit entry.
