Fetch raw SKILL.md for activation (lightweight endpoint)

Returns the raw SKILL.md content for a skill without the MCP protocol overhead. Used by the UserPromptSubmit hook during skill activation.

Auth: requires Authorization: Bearer sk_live_... but does NOT surface auth failures as 401. Missing/invalid/expired tokens, missing query params, skill-not-found, and skill-with-no-content ALL return the same 404 with {"error": "not_found"}. This is intentional — the route never leaks which failure mode applies, so the OpenAPI spec deliberately omits a security declaration. Callers MUST still provide a valid bearer token; anything else yields 404.

Unlike the other CLI routes this endpoint does NOT go through withAccessKey — it resolves the bearer token directly to preserve the failure-collapse semantics. Scopes are not enforced. Rate limits are not applied at the route level.
