Update Skill

Update a skill. A title, description, body, tool_allowlist, goal or attachment_instruction change (null clears either) creates a new immutable version; an omitted goal / attachment_instruction is carried forward. default_mode / is_archived only change the skill. Invalid goal → 422 invalid_goal (see create).
