Revise the integration_shape.current, logging the reason in revision_log.
patch: fields to merge into integration_shape.current (top-level keys like delivery, surfaces, per_goal_expectations, rationale, open_questions). reason: free-text explanation — required. Captured in revision_log so the MCP can detect drift from the Roxels-authored proposal.
Status transitions: proposed_by_roxels → under_revision on first revision. Use POST /integration-shape/confirm to mark as confirmed.
