Update a clinic program

Partially updates an active clinic program's name and/or facility association. At least one of name or facilityId must be provided; omitted fields are left unchanged. programType and levelOfCare cannot be updated via this endpoint. A malformed facilityId UUID returns 400. A missing or archived facilityId returns 404.
