Patch crew settings (human only, step-up, If-Match)

Human principal only (D27) with a login in the last 15 minutes. If-Match is the settings_version (412 on mismatch). Emits crew.settings_changed (a moment) and an audit row. require_report_for_done cannot be turned off (422).
