Mints the code on first call rather than at signup, so the accounts that predate the program need no backfill. Returns enabled: false with no code while the flag is off — minting links people can't be rewarded for would strand anyone who shared one early.

enabled: false is also the answer for a lifetime holder, decided in referral_summary: there is no recurring invoice for a free month to mean anything on, so every surface that keys off this flag — the sidebar card, this section and its nav row, the referral email preference, the promo modal — hides itself rather than advertising an offer they cannot take.
