Authentication: JWT required via Authorization header

Args: width: Target width (optional) height: Target height (optional) format: Output format (jpeg/webp/avif/png, default: webp) quality: Image quality 1-100 (default: 85)

Returns: Signed imgproxy URL with expiry

Raises: 404: Clinician profile not found or no profile picture 400: Invalid transformation parameters 500: URL generation failed
