Submit a campaign's questionnaire

Validates combined core-completeness across PROFILE + SHARED_CORE + (when eligible) STORY, then snapshots the effective answers and locks the questionnaire for edits. Rejects with a 400 listing unanswered_core when incomplete, and a 409 when already submitted and not since unlocked.
