Ad account lookup (deferred)

Unavailable for Facebook and Instagram while PostLake defers Meta advertising features. Returns HTTP 501 with code meta_ads_deferred, not an empty account list. Reconnecting or using an older token will not enable it. The former success schema is retained for client compatibility.
