Remove a user's MFA enrollment.

Changed on

Removes another user's MFA, for a user who has lost their device, and clears any lock. On the caller's own user, use POST /api/users/me/mfa/remove, which takes a code. Requires an administrator as well as the scope. Recorded as a user_mfa_removed audit event naming the calling administrator and API key. Requires the users:write scope.
