Update the account owner for an organization (internal)

Applies a partial update to the user holding the account_owner role for the supplied organization_id. Email changes for confirmed owners are staged via email_confirmations (link is sent to the new address); for unconfirmed owners the email column is written directly. Cross-user email uniqueness is enforced.
