Login

When code is omitted: validate credentials, then either issue an OTP challenge (LoginMFA on, response type "challenge") or issue tokens (LoginMFA off, response type "jwt"). When code is populated: validate credentials, verify the OTP, and issue tokens (response type "jwt").
