200 {outcome: placed, interaction_id, receipt} or {outcome: failed, reason, receipt, released, release_reason}. Refusals carry detail.reason and leave the contact untouched, except when a call prepared earlier for this run and never dialed was returned to awaiting_cortex: the refusal then carries released: true.
