Create Api Key Route

Mint a new PAT. The plaintext is returned exactly once in the response body — the UI must show it immediately and warn the user they won't see it again.

PAT issuance is plan-gated (the locked pricing makes the agent surface a paid feature): Creator/free = 0, Studio = 1, Operator = unlimited. We gate minting, not per-call — the token IS the capability. Existing keys on a now-ineligible plan are left intact (no retroactive lockout); only NEW issuance is blocked.
