Record Go Live Settings

Record the Payment step's delivery costs: the courier tip added to every delivery (the default_tip_cents preference) and the price limit on the default options group (null turns it off). It doesn't activate the organization; saving a payment method does. ORG_NOT_LAUNCHPAD (404) for an organization that didn't sign up through Launchpad.
