Admin Disable User Auth
Abuse kill switch (D-13/D-32): stamps revoked_at on the company's OIDC client. Structurally irreversible from every provisioning path — restore and enable_user_auth both refuse; only the operator un-revoke lever below clears it. revoked_reason + revoked_conglomerate_id survive the company row (D-55) so repeat offenders stay visible.
