Form submissions on a page, with their leads

Every form_submit on one page of the company's websites in a date window, each with the lead its visitor is linked to (Z_LeadVisitors) and that lead's name / email / phone — new contacts AND returning ones matched onto an existing lead.

One row per (submission, lead): a visitor linked to two leads (a shared device) repeats the same event_id once per lead. A submission whose visitor reaches no lead comes back with lead_id null.
