Publish or schedule a media URL to a specific connected social account.

If scheduled_at is omitted, publishes immediately. If scheduled_at is provided (ISO 8601), saves to the queue and returns {"status": "scheduled", "id": <post_id>, "scheduled_at": "..."}.
