Experimental local-index signal screen; not a compliance decision

Search by name or a mapped ticker, CIK, or UEI. NPI-only requests search just the local HHS OIG exclusion table. Name-based scores are heuristic; partial_results reports local query failures only, not full coverage or current upstream status. A zero score is not clearance.
