GitHub App webhook (pv2 console federation)

Receives GitHub App events. Authenticated by X-Hub-Signature-256 (HMAC-SHA256 of the raw body with GITHUB_APP_WEBHOOK_SECRET, timing-safe compared) — 401 invalid_signature otherwise, 503 github_app_unconfigured when the secret is unset. Handles installation (created/deleted/suspend/unsuspend/new_permissions_accepted) and installation_repositories (added/removed) for installations already bound to a Mnemom org; deleted also revokes every open token grant. Everything else is acknowledged and ignored. Handler: handleGithubWebhook in src/org/github-federation.ts.
