Paginated model-failover events

Reads model_failover_events: rows recorded at checkpoint-write time whenever the (provider, model) reported on a checkpoint differs from the immediately preceding checkpoint in the same session. Each row is strictly an OBSERVED provider/model change — this API has no visibility into why the change happened, so reason is always null today. Same owner-scoped read gate as GET /v1/agents/{agent_id}/checkpoints (no new authorization surface).
