Get Decision

Context & Intelligence Layer PRD §7: a decision as an independently fetchable object — the Room/Page "current decision" card. Also resolves whether a newer decision supersedes this one (via context_links' supersedes relationship, asTarget) — see 0028_decision_context_links.sql for why that's a derived link rather than a 4th status value on the decision itself.
