Pulse Confirm

Decision Pulse's "Confirm still real" — resets the decay clock (db_decisions.confirm_pulse) and immediately resolves any open decision_without_work insight tracking it, so Home stops nudging about it right away rather than waiting for the next insight_loop pass. See db/decisions.py::list_without_work_for_insight for how the clock is read.
