Save Attachment To Docs

The explicit, opt-in "Save to Docs" — chat attachments never become a Doc on their own any more (see 0025_chat_attachments.sql). Copies the same storage object into a new document row rather than re-uploading the bytes; context/doc_import.py's link_preview_doc builds the identical content shape routers/documents.py's own /upload would have produced for this file.
